What Is Hospice Care,
Who Needs It, And Why

Hospice care is a special type of health care that, typically, focuses on providing a certain quality of life for people who are experiencing a life-limiting illness. Provista’s hospice care focuses on treating the person and symptoms of the disease, rather than treating the disease itself. It includes the patient and their family in the decision-making process. In general, hospice care must be used when the person is expected to live around 6 months or less. A good case practice is that the person with the illness needs to have a discussion with his/her family members and doctor to decide when they should begin the hospice care.

All throughout the process, the team of hospice physicians, nurses, chaplains, medical workers, and other healthcare specialists at Provista Healthcare will be there with the family members and the patient. To qualify for hospice care at home you must meet the two qualifications below:

  1. Both the physicians, the attending physician, and the hospice medical director, must certify that the patient is terminally ill, with a six-month or less life expectancy if the disease takes its normal course.
  2. The patient and/or family must be aware of the prognosis and elect comfort care. I.e. consent of the patient and his/her family members.

Once you have signed up for hospice care, the first thing you need to do is call Provista Hospice care and set up an initial assessment of the patient’s needs. Any family member, the patient himself/herself, the doctor, or someone at the hospital can make the call.

Team members of the Provista hospice care team will pay you a visit and get you started. Our compassionate and dedicated team consist of: The hospice team – a physician, nurse, hospice aide, social worker, chaplain, volunteer, and bereavement manager, each one of them will schedule separate time to meet with you at the location of your choice. Any necessary home medical equipment will be delivered, and any necessary medications will be delivered too.
